Where to Find Us

Collaboration with the community is an essential feature of all our programs and services. Partners like Los Angeles Unified School District, USC Ostrow School of Dentistry, local churches and synagogues have helped us be that link to coordinating care for patients. Many times, our Health & Faith Partnership provides an entry point for many into our healthcare system, but it does not end there. We also want to be the bridge to health services with our trusted health partners and advocates.

Locations for our QueensCare Health & Faith Partnership Team

QueensCare parish nurses and community health workers (CHWs) are located at partner sites so members of the congregation, or the neighborhood, can meet with them on a walk-in basis. The parish nurses and CHWs provide low or no-cost immunizations and screenings to children and adults, as well as health education and counseling. Additionally, parish nurses refer those requiring further medical attention to healthcare resources within the community. Additional programs and services such as the QueensCare Mobile Dental and Mobile Vision Programs are not included on this list. These programs spend most of the school year stationed at LAUSD campuses. During summer and winter breaks, they host clinics at partner sites with appointments available to the public.
