This team of nurses has offered a range of preventative health screenings and health education materials personalized to patients’ needs, all on a walk-in basis. They work alongside Community Health Workers to connect patients with the resources they need, including referrals and advocacy.
Since 1992, the Health & Faith Partnership has created safe, personal access points to healthcare for underserved community members. Working through places of worship, schools, and social service agencies, dedicated parish and community nurses and Community Health Workers (CHWs) provide free health screenings and connect patients with essential health resources.
This approach addresses not just immediate health needs but also factors that impact overall well-being—from food security to housing stability. Through preventative screenings, immunizations, health education, and compassionate guidance, the program creates healthier communities one relationship at a time.
When patients visit one of the partner locations, they find healthcare professionals who see them as whole persons—not just a set of symptoms. For those needing additional care, the team provides warm referrals to community resources and often accompanies patients to appointments, ensuring no one navigates the healthcare journey alone.
